How to register:
Call into the surgery with· Either your NHS card and number plus details of your previous GP· or proof of ID (passport) and current address (a household bill) and complete a registration form.Seeing your GP:Appointments with either the Doctors or Nurse are in surgery, by appointment, up to 4 weeks in advance. Same day emergency appointments are always available. We try to ensure you can see the Doctor of your choice, subject to availability.Telephone consultations:Our receptionist will advise you of the best time to call for a telephone consultation, should this be required.Home visits:Home visits are available for the housebound. If required, please try to call before 10am.

If you have a complaint about any aspect of our service, please contact the Practice Manager.
If you do not receive a satisfactory answer, or wish to take the matter further, please put your complaint in writing, addressed to Dr K. Crisp.
